Makefile: always remake index.html
Current implementation doesn't update index.html even though menu
and/or subindex were updated.
Thus, browser uses old index.html, and we will miss new menu.
This patch always remakes index.html to solve it.
This patch also use "ln -sf" instead of "ln -f" for it.
